According to Item 7 of the Regulations on the State Customs Service of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of 18.07.2007 of N 940, and in pursuance of Item 10 of the Regulations on the cargo customs declaration, approved by the resolution of the Cabinet of Ministers of Ukraine of 09.06.97 of N574 (with changes), I ORDER:

1. Approve samples of forms of the single administrative document of the MD-2 form and additional leaf of the MD-3, form of amendment to it to the cargo customs declaration of the MD-6, form of the single administrative document of the MD-4 form and additional leaf of the MD-5, form of amendment to it to the cargo customs declaration of the MD-7 form and the specification of the MD-8, form which are applied.

2. Approve the Technical description on forms of the single administrative document of the MD-2 form and additional leaf of the MD-3, form of amendment to it to the cargo customs declaration of the MD-6, form of the single administrative document of the MD-4 form and additional leaf of the MD-5, form of amendment to it to the cargo customs declaration of the MD-7 form and the specification of the MD-8, form which is attached.

3. To department of declaring and customs regimes (Naumenko V. P.) and Legal department (Not brother L.M.) provide provision of this order on state registration in the Ministry of Justice of Ukraine.

4. To regional information customs (Koposov S.A.) till 01.01.2008 to finish the software taking into account Items 1 and 2 of this order.
