1. Approve reporting forms (administrative data) N7-HK "Activities Report of Club Institutions", N7-HK (summary) "The summary report on activities of club institutions" and the Instruction concerning their filling which are attached.
2. Extend the N7-HK, N7-HK forms (summary) and Instructions concerning their filling to the club institutions located in the territory of Ukraine irrespective of subordination.
3. Enter the N7-HK and N7-HK forms (summary) into action, starting with the report for 2007.
4. Establish the following procedure for submission of the reporting under the N7-HK, N7-HK forms (summary):
