About approval of the Regulations on procedure for registration and issue of the insert in the certificate of birth confirming availability at the child of nationality of the Kyrgyz Republic
For the purpose of implementation of the Agreement between the Government of the Kyrgyz Republic and the Government of the Republic of Uzbekistan about mutual trips of citizens signed on October 3, 2006 in the city of Tashkent and also creating favorable conditions for crossing of frontier between the Kyrgyz Republic and the Republic of Uzbekistan and stay in the Republic of Uzbekistan of citizens of the Kyrgyz Republic the Government of the Kyrgyz Republic decides:
1. Approve enclosed:
2. In accordance with the established procedure to provide to the Ministry of Internal Affairs of the Kyrgyz Republic production of forms of the insert in the certificate of birth confirming availability at the child of nationality of the Kyrgyz Republic.
3. To impose control over the implementation of this Resolution on the Ministry of Internal Affairs of the Kyrgyz Republic and the Ministry of Foreign Affairs of the Kyrgyz Republic.
4. Publish this Resolution in mass media.

The insert in the certificate of birth confirming availability at the child of nationality of the Kyrgyz Republic (further - the insert) is the document (together with the certificate of birth) granting the right to the citizen of the Kyrgyz Republic who did not reach 16-year age on crossing of frontier between the Kyrgyz Republic and the Republic of Uzbekistan and stay in the Republic of Uzbekistan.
The insert represents the book, 115x80 mm in size, consisting of 12 pages executed on special paper of light yellow color with watermarks. Texts and razgrafleniya are executed by paint of black color. Sheets are stuck together, all pages are numbered. The first page of the insert after filling is laminated by film.

1. This Provision is developed according to the Agreement between the Government of the Kyrgyz Republic and the Government of the Republic of Uzbekistan on mutual trips of citizens signed on October 3, 2006 in the city of Tashkent.
2. The provision determines procedure for registration and issue of the insert in the certificate of birth confirming availability at the child of nationality of the Kyrgyz Republic (further - the insert).
3. The insert is issued to the certificate of birth for the citizens of the Kyrgyz Republic who did not reach 16-year age and is the document granting the right for entrance, departure, transit and stay in the territory of the Republic of Uzbekistan in case of obligatory escort of this person by the parent or legal representative.
4. Acceptance, registration and issue of the insert to citizens of the Kyrgyz Republic are performed:
- in the territory of the Kyrgyz Republic - Management of passport and visa control of the Ministry of Internal Affairs of the Kyrgyz Republic (further - UPVK Ministry of Internal Affairs) or its territorial subdivisions;
- outside the Kyrgyz Republic - foreign organizations of the Ministry of Foreign Affairs of the Kyrgyz Republic (further - the MFA).
5. The insert without certificate of birth is invalid.
6. The insert is made in the form approved by the Government of the Kyrgyz Republic on special paper with protective elements and has 8 pages.
7. The insert is filled in in the state and official languages and contains the following data:
- surname, name, middle name;
- series and certificate number about the birth;
- name of body of issue;
- date of issue.
Also adjusting data of the accompanying person (accompanying persons) are entered:
- surname, name, middle name;
- birth date;
- passport number;
- who issues the passport.
It is forbidden to make in the insert of record, not provided by this Provision.
8. In the insert the picture of black-and-white color, 35x45 mm in size is pasted, the seal and the signature of the official who issued the document are set.
9. For receipt of the insert the parents of the child or his legal representatives living in the territory of the Kyrgyz Republic address in UPVK Ministry of Internal Affairs or to its territorial subdivisions at the place of residence, with provision of the following documents:
- the application written in any form, signed by one of parents or the legal representative;
- the certificate of birth (returns to the owner);
- passport copies of parents or legal representatives;
- photo of 35x45 mm in size;
- copies of notarially certified consent of parents or legal representatives to the right of escort of the child.
The parents of the child or his legal representatives living outside the republic for receipt of the insert address to foreign organizations the MFA.
10. The statement on receipt of the insert is taken cognizance only after provision of all necessary documents specified in Item 9 of this provision.
11. The insert is drawn up for a period of up to 6 months and issued within 3 working days from the date of adoption of documents.
12. Registration of forms of the insert is made in register of statements. According to the same magazines it is recorded expense of forms of the insert. Registers shall be strung together, numbered, under seal also the signature of persons, responsible for storage. Magazines are filled in accurately, without blots.
13. In case of detection of excesses, shortage or the damaged forms of the insert, the body, specified in item 4 of this provision, draws up the statement in duplicate (the 1st copy of the act goes to UPVK Ministry of Internal Affairs, the 2nd copy remains at the sender).
14. Forms of the insert are forms of the strict reporting.
Forms of the insert are stored in specially equipped rooms, separate fireproof safes or fireproof cases which shall be sealed.
15. Materials on receipt of the insert are filed in nomenclature cases. Nomenclature cases are stored during 1 year in the rooms ensuring their safety.
16. Forms of the insert and statements, the magazines provided by this Provision are provided to UPVK Ministry of Internal Affairs and MFA.
17. The invalid, unclaimed, and also subject to exchange forms of the insert are destroyed by burning or cutting of UPVK Ministry of Internal Affairs or MFA by the relevant commission.
