In pursuance of the Law of Ukraine "About social services" and Articles 5, of 6, of 8, 9 Laws of Ukraine "About licensing of certain types of economic activity", resolutions of the Cabinet of Ministers of Ukraine, of 26.04.2007 N 667 "About approval of the Regulations on the State committee of Ukraine concerning regulatory policy and entrepreneurship", of 02.11.2006 N 1543 "About approval of the Regulations on the Ministry of Labour and Social Policy of Ukraine", of 14.11.2000 N 1698 "About approval of the list of bodies of licensing" we ORDER:

1. Approve Licensed conditions of implementation of professional activity in the field of provision of social services and the Procedure for control over the implementation of Licensed conditions of implementation of professional activity in the field of provision of social services which are applied.

2. To department of licensing of the State committee of Ukraine concerning regulatory policy and entrepreneurship (O. Ivanchenko), to Department for veterans and people of advanced years of the Ministry of Labour and Social Policy of Ukraine (O. Krentovskaya) in accordance with the established procedure to provide submission of this order on state registration in the Ministry of Justice of Ukraine.

3. To provide to the deputy minister of work and social policy of Ukraine S. Bychkov the publication of this order in mass media.

4. To impose control over the implementation of this order on the vice-chairman of the State committee of Ukraine concerning regulatory policy and entrepreneurship M. Pristupa and the deputy minister of work and social policy of Ukraine S. Bychkov.

Licensed conditions of implementation of professional activity in the field of provision of social services

I. General provisions

1.1. Licensed conditions of implementation of professional activity in the field of provision of social services (Licensed conditions are farther) are developed according to the laws of Ukraine "About social services", "About licensing of certain types of economic activity", resolutions of the Cabinet of Ministers of Ukraine of 14.11.2000 N 1698 "About approval of the list of bodies of licensing", of 04.07.2001 N 756 "About approval of the list of documents which are enclosed to the application for licensing for separate type of economic activity".

1.2. These Licensed conditions establish the exhaustive list of the organizational, qualification and other special requirements obligatory for accomplishment when implementing professional activity in the field of provision of social services.

1.3. In these Licensed conditions terms and determinations are used in the values provided by the Laws of Ukraine "About social services" and "About licensing of certain types of economic activity".

1.4. Bodies of licensing of implementation of professional activity in the field of provision of social services is the Ministry of Labour and Social Policy of Ukraine, Council of Ministers of the Autonomous Republic of Crimea, the regional, Kiev and Sevastopol city state administrations.

1.5. The subject of managing which intends to conduct professional activity in the field of provision of social services personally or through the body authorized by it or person files in relevant organ of licensing petition of the established sample for licensing (appendix) and the relevant documents, the stipulated in Article 10 Laws of Ukraine "About licensing of certain types of economic activity".

1.6. The license is granted for implementation of professional activity in the field of provision of complex of social services which include such types:

social services;

psychological services;

social and pedagogical services;

social and medical services;

social and economic services;

legal services;

services in employment;

services in employment rehabilitation of persons physically challenged;

information services;

other social services.
