In pursuance of the resolution of the Cabinet of Ministers of the Azerbaijan Republic "About approval of the Procedure for assignment and application of identification taxpayer number" of December 21, 2000 I order to No. 222:

1. Approve forms of the following documents (are applied):

* Data on branches and other business entities (objects) of the taxpayer;

* Data on branches and (or) other business entities (objects) of the taxpayer staying on the registry in Tax Administration (department);

* The magazine of registration of branches and other business entities (objects) of the taxpayers staying on the registry in other territorial tax authorities;

* Magazine of registration of certificates;

* The statement on receipt of the certificate duplicate;

* Certificate duplicate;

* Magazine of registration of certificates duplicates;

* Statement for modification of information about the taxpayer;

* The magazine of registration of the liquidated taxpayers.

2. Charge to the Ministry of taxes Nakhchivan ARE, to the Head Tax Administration on the city of Baku, to provide to Tax Administrations (departments) of the cities and areas since January 1, 2001 use of the forms approved by this order in case of assignment and application of identification taxpayer number.

3. Charge to the Chief of the Secretariat to provide (E. Gadzhiyev) bringing copies of this order to all structural divisions of the Ministry of taxes.

4. Charge to Financial and economic management to take (R. Latifov) measures to providing tax authorities with forms of the established forms.
