According to the Order of registration of the organizations which activities are directed to requirements satisfaction of subjects of small and medium entrepreneurship, the approved resolution of the Cabinet of Ministers of Ukraine of 21.05.2009 of N 510, I ORDER:

1. Approve enclosed:

the application form for carrying out registration of the organization as infrastructure facility of support of small and medium entrepreneurship;

form of the Registration certificate of the organization as infrastructure facility of support of small and medium entrepreneurship;

form of the register of infrastructure facilities of support of small and medium entrepreneurship;

form of the book of document registration which move the organizations for their registration as infrastructure facilities of support of small and medium entrepreneurship.

2. To development department of entrepreneurship (Nagrabovy O. O.):

2.1. Provide submission of this order on state registration in the Ministry of Justice of Ukraine.

2.2. Bring this order to permission and management in work of territorial authorities of the State Committee of Ukraine on Regulatory Policy and Entrepreneurship in the Autonomous Republic of Crimea, areas, the cities of Kiev and Sevastopol.
