About approval of forms of the reporting under questions of civil registration and the Instruction about accounting of civil registration by departments

According to article 15 of the Law of Ukraine "About registration authorities of acts of civil status" and articles 13 and 16 of the Law of Ukraine "About the state statistics" I ORDER:

1. Approve reporting forms which are applied:

N 97 "About civil registration" (monthly);

N 98 "About civil registration" (semi-annual, annual).

2. Approve the Instruction about accounting of civil registration by departments which is applied.

3. Determine that collection and the data of administrative data on civil registration is performed by departments of civil registration district, district in the cities, city (the cities of regional value), gorrayonny justice departments, Head department of justice of the Ministry of Justice of Ukraine in the Autonomous Republic of Crimea, head departments of justice in areas, the cities of Kiev and Sevastopol and the Ministry of Justice of Ukraine.

4. To department for civil status of citizens (Kund V. V.) to bring this order to permission of Head department of justice of the Ministry of Justice of Ukraine in the Autonomous Republic of Crimea, head departments of justice in areas, the cities of Kiev and Sevastopol.

5. This order becomes effective from 01.01.2010.
