About approval of forms of the reporting under questions of state registration of acts of civil status and the Instruction about accounting of state registration of acts of civil status by departments

According to article 10 of the Law of Ukraine "About state registration of acts of civil status" and Articles 13, 16 Laws of Ukraine "About the state statistics" ORDER:

1. Approve reporting forms which are applied:

No. 97 "Report on state registration of acts of civil status" (monthly);

No. 98 "Report on state registration of acts of civil status" (semi-annual, annual).

2. Approve the Instruction about accounting of state registration of acts of civil status by departments which is applied.

3. Declare invalid the order of the Ministry of Justice of Ukraine of October 12, 2009 No. 1822/5 "About approval of forms of the reporting under questions of civil registration and the Instruction about accounting of civil registration by departments", registered in the Ministry of Justice of Ukraine on October 12, 2009 for No. 949/16965.

4. Charges and data of administrative data on state registration of acts of civil status are performed by departments of state registration of acts of civil status of interregional managements of the Ministry of Justice of Ukraine and departments of state registration of acts of civil status of managements of state registration of interregional managements of the Ministry of Justice of Ukraine.
