1. Approve:

form of the Tax declaration of results of joint activities in the territory of Ukraine without creation of the legal entity (further - the Tax declaration) which is applied;

Procedure for accounting of results of joint activities in the territory of Ukraine without creation of the legal entity which is applied.

2. Declare invalid the order of State Tax Administration of Ukraine of 28.02.2011 No. 113 "About approval of form of the Tax declaration of results of joint activities in the territory of Ukraine without creation of the legal entity and the Procedure for accounting of results of joint activities in the territory of Ukraine without creation of the legal entity", registered in the Ministry of Justice of Ukraine 29.03.2011 for No. 412/19150.

3. To department of tax, customs policy and methodology of financial accounting of the Ministry of Finance of Ukraine (Chmeruk M. O.), to Department of the taxation of legal entities of the State Tax Administration of Ukraine (Lyubchenko O. M.) in accordance with the established procedure to provide:
