1.2. Form of primary accounting documentation No. 123-1/o "Card about the state of health of applicants for the positions connected with accomplishment of functions of the state or local self-government" (further - form No. 1231/about) and the instruction for its filling (are applied).

1.3. Form of primary accounting documentation No. 133/about "The medical certificate about the state of health of applicants for the positions connected with accomplishment of functions of the state or local self-government" (further - form No. 133/about) and the instruction for its filling (are applied).
