About approval of standards of provision of services of social support to the population by the principle of "single window"

On accomplishment of the order of the Cabinet of Ministers of Ukraine of February 03, 2012 and for the purpose of streamlining of system of provision of services of social support to the population by the principle of "single window" and exit work of "mobile social office" I ORDER to No. 4959/0/1-12:

1. Approve enclosed:

1.1. The standard of provision of service to destination compensations to persons who have the right to free receipt of coal to household requirements, but live in the houses having central heating.

1.2. The standard of provision of service to destination privileges for acquisition of the liquefied gas to persons which have the right to privileges.

1.3. The standard of provision of service to destination compensations on acquisition of solid and liquid household fuel to persons who have the right to privileges.

1.4. The standard of provision of service to destination compensations to physical persons which provide social services.

1.5. The standard of provision of service to destination and payment of one-time remuneration to women which gives honorary title of Ukraine "Mother heroine".

1.6. The standard of provision of service in certification of parents and the child from large family.

1.7. The standard of provision of service to destination monthly compensation payment to unemployed person who performs care of the disabled person of the I group or of person which reached 80-year age.

1.8. The standard of provision of service to destination and payment of the government public assistance on leaving.

1.9. The standard of provision of service to destination and payment of the government public assistance to persons who have no pension entitlement, and to disabled people.

1.10. The standard of provision of service to destination compensation payments to citizens which suffered as a result of the Chernobyl catastrophic crash.

1.11. The standard of provision of service to destination and payment to some categories of disabled people of monetary compensation instead of the sanatorium permit and cost of independent sanatornokurortny treatment.

1.12. Standard of provision of service to destination and payment of monetary compensation of cost of sanatorium treatment to some categories of citizens.

2. To provide to social services department (I. Tarabukin) submission of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine.

To the minister of social policy of the Autonomous Republic of Crimea, chiefs of head departments of work and social protection of the population regional, Head department of work and social protection of the population Kiev city, Managements of work and social protection of the population Sevastopol city public administrations to provide monitoring and control of observance of standards of provision of services of social support of the population by the principle of "single window".

The standard of provision of service to destination compensations to persons who have the right to free receipt of coal to household requirements, but live in the houses having central heating

І. General provisions

1.1. This Standard No. 887 "About approval of the Procedure for provision of compensation to persons who have the right to free receipt of coal to household requirements is developed according to Articles 43, 48 Mountain laws of Ukraine, the resolution of the Cabinet of Ministers of Ukraine of August 12, 2009, but live in houses with central heating".

1.2. This Standard determines the mechanism of purpose of compensation for payment of the electric power, gas and central heating of housing to persons who have the right to its obtaining according to Articles 43, 48 Mountain laws of Ukraine (further - compensation).

1.3. The administrative authority providing service - managements of work and social protection of the population of district, district public administrations in the cities of Kiev and Sevastopol, structural divisions concerning work and social protection of the population of executive bodies city, district in the cities (except the cities of Kiev and Sevastopol) councils (further - authorized body).

ІІ. List of categories of receivers of service

Receivers of service are persons who do not work at the coal companies, have the right to free receipt of coal to household requirements, but 48 Mountain laws of Ukraine live in the houses having central heating according to Articles 43,.

ІІІ. List of the documents necessary for provision (obtaining) of service

3.1. For purpose of compensation of person, not working at the coal companies, submit the application which form is approved by the order of the Ministry of social policy of Ukraine of February 22, 2012 No. 96 "About approval of the application form about purpose of all types of the public assistance, compensations, subsidies and privileges", registered in the Ministry of Justice of Ukraine on March 20, 2012 for No. 418/20731, in which account details in authorized bank on which compensation is listed are specified, and show the service record and the pension certificate (except not employed workers) which confirm compliance to requirements of articles 43 and 48 of the Mountain law of Ukraine.

3.2. Are enclosed to the application:

3.2.1 data on security with living space and utilities in the form approved by Council of Ministers of the Autonomous Republic of Crimea, the regional, Kiev and Sevastopol city public administrations;

3.2.2 the copy of the certificate of assignment of registration number of accounting card of the taxpayer or series and passport number with mark about refusal of adoption of such number;

3.2.3 the certificate of stay on accounting in jobcenter (for not employed workers);

3.2.4 the conclusion of medico-social examination (for disabled people);

3.2.5 the copy of the certificate on the right of carrying the signs "Miner's Glory" or "Miner's Valour" I, ІІ, ІІІ degrees (for persons awarded with such signs);

3.2.6 the copy of the certificate of the disabled person or veteran of war and work (for disabled people and veterans of war and work);

3.2.7 the act of the accident connected with production in the H1 form according to appendix 4 to the Procedure for conducting investigation and accounting of accidents, occupational diseases and accidents on production approved by the resolution of the Cabinet of Ministers of Ukraine of November 30, 2011 No. 1232 (for families of workers, the died (dead) at the companies for production (conversion) of coal who receive pension in connection with loss of the supporter).

ІV. Place and procedure for representation of request for receipt of service

4.1. For purpose of compensation persons address to authorized bodies by the principle of "single window" or to exit "mobile social office".

4.2. Time of giving of request for receipt of service: according to the approved working schedule of authorized body or "mobile social office".

4.3. Priority of giving of request for receipt of service: in reception hours according to the working schedule of authorized body or "mobile social office".

V. Structure and sequence of actions of the receiver and authorized body, description of stages of provision of service

5.1. Persons submit the application and documents to it determined by the Section ІІІ this Standard, according to the procedure, established by the Section ІV of this Standard.

5.2. Documents acceptance is performed by the acceptance principle specialist of "single window" of authorized body or "mobile social office".

5.3. The acceptance specialist conducts interview with the applicant and adopts from it necessary document package and the statement which registers in the magazine of registration of statements.

5.4. The acceptance specialist during documents acceptance:

checks correctness of filling of the application blank and completeness of the documents necessary for purpose of compensation;

issues to the applicant the notification (the detachable coupon, the receipt) on adoption of the statement and documents.

5.5. In case to the statement not all necessary documents are provided, the acceptance specialist provides explanations what documents shall be filed to the statement.

5.6. Compensation is charged based on the shown payment documents (the pay books, payment receipts) on the actual payment consumed for the last month of the electric power, natural gas for cooking and services in central heating of housing which move monthly to the 25th of the next period.

Receivers of compensation who on reasonable excuses (business trip, disease, the death of the loved one and so forth) timely did not file the specified payment documents can give them within two next months.

5.7. Compensation is paid monthly through authorized banks.

The amount of compensation is calculated as payment amount for services in central heating of housing, electric utility service and gas supply (taking into account privileges which are provided according to the legislation) within regulations of ownership or use of living space and regulations of electric power consumption and natural gas for cooking taking into account which the subsidy for expense recovery for payment of housing and communal services is appointed.

5.8. The authorized body after receipt of budgetary funds for its account opened in territorial authority of the Public treasurer service of Ukraine lists them based on payable documents (sheets) to authorized banks for compensation payment.
