1. Approve:

1.1. Procedure for write-off of penalties and penalty fee to the payers who satisfied terms of the contract about payment by installments of outstanding amounts on payment of insurance premiums which is applied.

1.2. Form of the agreement on payment by installments of outstanding amounts on payment of insurance premiums which is applied.

2. To department of receipt of the income (Litvinenko V. V.) and legal management (Ryabtseva T. B.) submit this resolution on state registration to the Ministry of Justice of Ukraine.

3. This resolution becomes effective from the date of its official publication.

4. To impose control over the implementation of this resolution on the First Deputy Chairman of the board Kravchuk V. O.
