About approval of forms of the reporting on availability and movement of material values of the state reserve

According to the Law of Ukraine "About the state material reserve", the subitem 6 of item 4 of the Regulations on the Ministry of Economic Development and Trade of Ukraine approved by the Presidential decree of Ukraine of May 31, 2011 No. 634, Item 23 of the Procedure for forming, placement and carrying out transactions with material values of the state reserve approved by the resolution of the Cabinet of Ministers of Ukraine of October 08, 1997 No. 1129, for the purpose of increase in overall performance of the state companies which belong to system of the state reserve in predpriyatiyotvetstvenny keepers and optimization of accounting of availability of material values of the state reserve I ORDER:

1. Approve such forms of the reporting which are applied:

form No. 1 "Report on availability of material values of the state reserve";

form No. 2 "Information concerning conversion of grain and realization of flour";

form No. 3 "Information concerning load of reservoir for storage of grain and grain movement";

form No. 4 "Information of rather actual availability of grain, including grain of the state reserve".

2. To department of economy of defense and (Nebotov P. G.) in accordance with the established procedure to ensure safety submission of this order on state registration in the Ministry of Justice of Ukraine.

3. This order becomes effective from the date of its official publication.

4. To impose control over the implementation of this order on the Chairman of the State agency of reserve of Ukraine Lelyuk O. V.
