1. To chiefs of managements of health care of areas, cities of Astana and Almaty (under approval):

1) to organize conducting personal accounting of persons injured with activities of the Semipalatinsk test nuclear test site, to designate the responsible person for execution;

2) to provide provision of the completed registration cards in the Republican state state company "scientific research institute of Radiation medicine and ecology" of the Ministry of Health of the Republic of Kazakhstan (further - Scientific Research Institute RMIE) all organizations of health care monthly to the 5th day of the next month.

2. Approve:

1) the "Registration Card of the State Scientific Automated Medical Register" form (further - GNAMR) according to appendix 1 to this order;

2) the Instruction for carrying out single registration of citizens of the Republic of Kazakhstan in the State scientific automated medical register of persons injured with activities of the Semipalatinsk test nuclear test site (further - the Instruction) according to appendix 2 to this order.

3. Scientific Research Institute RMIE (Apsalikov K. N.), together with heads of department of health care of the East Kazakhstan, Karaganda, Pavlodar regions (under approval):
