1. Approve on representation of the Ministry of Housing and Public Utilities of the Republic of Belarus enclosed:
1.1. form of the state statistical reporting 4-secondary raw materials (Minzhilkomkhoz) "Report on procurement and supply of secondary raw materials" of quarter frequency and to enact it starting with the report for January-September, 2012;
1.2. Instructions for filling of form of the state statistical reporting 4 secondary raw materials (Minzhilkomkhoz) "Report on procurement and supply of secondary raw materials" and to enact them starting with the report for January-September, 2012.
2. Extend the form of the state statistical reporting specified in subitem 1.1 of Item 1 of this resolution on the legal entities performing procurement or procurement and conversion of secondary raw materials.
3. This resolution becomes effective in fifteen working days after its signing.
