Based on the Regulations on National Statistical Committee of the Republic of Belarus approved by the Presidential decree of the Republic of Belarus of August 26, 2008 No. 445 "About some questions of bodies of the state statistics" the National Statistical Committee of the Republic of Belarus DECIDES:
1. Approve on representation of the Ministry of Agriculture and Food of the Republic of Belarus enclosed:
1.1. form of the state statistical reporting of 12-vetbolezna (Ministry of Agriculture and Food Production) "The report on infectious diseases of animals" of monthly frequency and to enact it starting with the report for January, 2013;
1.2. Instructions for filling of form of the state statistical reporting 12vetbolezni (Ministry of Agriculture and Food Production) "Report on infectious diseases of animals" and to enact them starting with the report for January, 2013.
2. Extend the form of the state statistical reporting specified in subitem 1.1 of Item 1 of this resolution on legal entities, the separate divisions of legal entities having separate balance, performing veterinary activities.
3. Recognize invalid:
the resolution of the Ministry of statistics and the analysis of the Republic of Belarus of October 22, 2007 No. 356 "About approval of form of the state statistical reporting 12-vt diseases (Ministry of Agriculture and Food Production) "The report on infectious diseases of animals" and instructions for its filling";
subitem 1.5 of Item 1 of the resolution of National Statistical Committee of the Republic of Belarus of October 21, 2008 No. 331 "About modification and amendments in some resolutions of the Ministry of statistics and the analysis of the Republic of Belarus";
subitem 1.22 of Item 1 of the resolution of National Statistical Committee of the Republic of Belarus of July 14, 2009 No. 84 "About modification of some forms of the state statistical reporting".
4. This resolution becomes effective since January 17, 2013.
