Based on the Regulations on National Statistical Committee of the Republic of Belarus approved by the Presidential decree of the Republic of Belarus of August 26, 2008 No. 445 "About some questions of bodies of the state statistics" the National Statistical Committee of the Republic of Belarus DECIDES:
1. Approve on representation of the Ministry of Agriculture and Food of the Republic of Belarus enclosed:
1.1. form of the state statistical reporting 2 veterinary supervision (Ministry of Agriculture and Food Production) "Report on veterinary supervision and veterinary and sanitary examination" of semi-annual frequency and to enact it starting with the report for January-June, 2013;
1.2. Instructions for filling of form of the state statistical reporting 2 veterinary supervision (Ministry of Agriculture and Food Production) "Report on veterinary supervision and veterinary and sanitary examination" and to enact them starting with the report for January-June, 2013.
2. Extend the form of the state statistical reporting specified in subitem 1.1 of Item 1 of this resolution on the legal entities exercising veterinary supervision except the legal entities performing conversion of meat of the cattle and bird.
3. Recognize invalid:
the resolution of the Ministry of statistics and the analysis of the Republic of Belarus of October 22, 2007 No. 335 "About approval of form of the state statistical reporting 2-vt supervision (Ministry of Agriculture and Food Production) "Report on veterinary supervision and veterinary and sanitary examination" and instructions for its filling";
subitem 1.3 of Item 1 of the resolution of National Statistical Committee of the Republic of Belarus of October 21, 2008 No. 331 "About modification and amendments in some resolutions of the Ministry of statistics and the analysis of the Republic of Belarus";
subitem 1.20 of Item 1 of the resolution of National Statistical Committee of the Republic of Belarus of July 14, 2009 No. 84 "About modification of some forms of the state statistical reporting";
the resolution of National Statistical Committee of the Republic of Belarus of September 24, 2010 No. 210 "About introduction of amendments to the resolution of the Ministry of statistics and the analysis of the Republic of Belarus of October 22, 2007 No. 335".
4. This resolution becomes effective since January 11, 2013.
