About approval of the Procedure for maintaining the register, forms of the registration statement, plea of nullity of registration and the registration certificate of the subjects of the industry of program products applying features of the taxation

For the purpose of ensuring implementation of requirements of the Law of Ukraine of July 05, 2012 "About modification of the Section XX "Transitional provisions" of the Tax code of Ukraine concerning features of the taxation of subjects of the industry of program products" I ORDER to No. 5091-VI:

1. Approve the Procedure for maintaining the register of the subjects of the industry of program products applying features of the taxation which is applied.

2. Approve applied:

form of the registration statement of the subject of the industry of program products as the subject applying features of the taxation;

form of the registration certificate of the subject of the industry of program products as the subject applying features of the taxation;

the application form about cancellation of registration of the subject of the industry of program products as the subject applying features of the taxation.

3. Recommend to subjects of the industry of program products, submitted before entry into force of this order registration statements for receipt of the Registration certificate of the subject of the industry of program products as the subject applying features of the taxation to submit such applications in the form approved by this order.
