Bring in the resolution of National Statistical Committee of the Republic of Belarus of July 29, 2011 No. 195 "About approval of form of the state statistical reporting 4-t (heads) "The wages report of heads" and instructions for its filling" (The national register of legal acts of the Republic of Belarus, 2011, No. 92, 7/1564; 2012, No. 21, 7/1710) following amendments:

1.1. add form of the state statistical reporting 4-t (heads) "The wages report of heads", approved by this resolution, after the detail "Name of Form" with the detail "Representation Type" of the following content:

1.2. in Instructions on filling of form of the state statistical reporting 4-t (heads) "The wages report of the heads" approved by this resolution:

to add part one of subitem 1.1 of Item 1 with words ", the Operativnoanalitichesky center in case of the President of the Republic of Belarus";

add with Item 1-1 of the following content:
