1. Approve:

the list of state bodies (licensors), representatives to grant licenses, on the types of activity which are subject to licensing according to appendix 1;

samples of forms of the license and appendix to the license according to appendices 2 and 3.

the list of the licensed types of activity on which the conclusion of bodies of sanitary, ecological, mining inspection and Gosenergonadzor, according to appendix 4 is necessary;

forms of statements for receipt of the license according to appendices 5 and 6.

1-1. The state coordinating body in the field of enhancement of system of licensing to determine the Ministry of Economy and Budget Planning of the Republic of Kazakhstan.

2. To the state bodies specified in appendix 1 to this resolution in case of development of procedure and conditions of licensing of specific types of the licensed activities, stipulated in Item 2 resolutions of the Cabinet of Ministers of the Republic of Kazakhstan of August 16, 1995 No. 1127 "About approval of the Regulations on payment procedure of the license fee for the occupation right separate types of activity" to determine:

qualification requirements to the licensed types of activity;

the list of the work types and services which are part of the licensed types of activity;
