Based on Item paragraph two 2, of paragraph two of Item 3 and part two of Item 10 of Article 7, of paragraph two of Item 2 of article 8 of the Law of the Republic of Belarus of July 12, 2013 No. 58-Z "About mediation", parts one of Item 12 of the Regulations on the Ministry of Justice of the Republic of Belarus approved by the resolution of Council of Ministers of the Republic of Belarus of July 26, 2024 No. 545, the Ministry of Justice of the Republic of Belarus DECIDES:
1. Establish:
the application form about state registration of the organization providing carrying out the mediation created in the form of organization according to appendix 1;
the application form about state registration of the changes and (or) additions made to articles of organization, the mediation providing carrying out created in the form of organization according to appendix 2;
the application form about liquidation of the organization providing carrying out the mediation created in the form of organization according to appendix 3;
the application form about registration of separate division of the legal entity providing carrying out mediation according to appendix 4;
form of register of legal entities in whom the separate divisions providing carrying out mediation according to appendix 5 are created.
2. This resolution becomes effective since January 24, 2014.
