According to the subitem 2) of article 4 of the Law of the Republic of Kazakhstan of October 11, 2011 "About religious activities and religious associations", the subitem 2) of Item 3 of article 16 of the Law of the Republic of Kazakhstan of March 19, 2010 "About the state statistics" and the subitem 11 of Item 15 of the Provision of Committee on cases of religions of the Ministry of Culture and sport of the Republic of Kazakhstan approved by the order of the Minister for Culture and Sports of the Republic of Kazakhstan of October 7, 2014 No. 30, I ORDER:

1. Approve the enclosed forms intended for collection of administrative data:

1) data on the number of religious associations, their branches, cult constructions and missionaries according to appendix 1 to this order;

2) data on cult buildings (constructions) and other rooms used by religious associations and their branches according to appendix 2 to this order;

3) the information about missionaries according to appendix 3 to this order;

4) data on special stationary rooms for distribution of religious literature and other information materials of religious content, objects of religious appointment according to appendix 4 to this order;

5) data on the spiritual (religious) organizations of education according to appendix 5 to this order;

6) data on rooms for holding religious actions outside cult buildings (constructions) according to appendix 6 to this order;

7) data on administrative offenses of the legislation in the field of religious activities according to appendix 7 to this order.

2. To provide to committee on cases of religions of the Ministry of Culture and sport of the Republic of Kazakhstan in the procedure established by the legislation:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) the direction of this order on official publication in periodic printing editions and information system of law of Ad_let within ten calendar days after its state registration;

3) placement of this order on Internet resource of the Ministry of Culture and sport of the Republic of Kazakhstan after its official publication.

4) bringing this order to data of all structural divisions of Committee on cases of religions of the Ministry of Culture and sport of the Republic of Kazakhstan and managements for religions of akimats of areas, the cities of Astana and Almaty.

3. To impose control of execution of this order on the vice-chairman of Committee on cases of religions of the Ministry of Culture and sport of the Republic of Kazakhstan Kulekeev B. K.

4. This order becomes effective after ten calendar days after day of its first official publication.
