1. Approve on representation of the Ministry of Health of the Republic of Belarus enclosed:
1.1. form of the state statistical reporting 12 infections (Ministry of Health) "Report on separate infectious, parasitic diseases and their carriers" of monthly frequency and to enact it starting with the report for January, 2016;
1.2. Instructions for filling of form of the state statistical reporting 12 infections (Ministry of Health) "Report on separate infectious, parasitic diseases and their carriers" and to enact them starting with the report for January, 2016.
2. Extend the form of the state statistical reporting specified in subitem 1.1 of Item 1 of this resolution to the district, zone, city centers of hygiene and epidemiology subordinated to the Ministry of Health of the Republic of Belarus, public institution "Center of hygiene and epidemiology" of Administration of the President of the Republic of Belarus, the organization of hygiene and epidemiology, subordinated to the Ministry of Defence of the Republic of Belarus, the Ministry of Internal Affairs of the Republic of Belarus, Committee for State Security of the Republic of Belarus, the State boundary committee of the Republic of Belarus.
3. Recognize invalid:
the resolution of the Ministry of statistics and the analysis of the Republic of Belarus of November 1, 2007 No. 374 "About approval of form of the state statistical reporting 6 infections (Ministry of Health) "Report on separate infectious, parasitic diseases and their carriers" and instructions for its filling";
the resolution of National Statistical Committee of the Republic of Belarus of October 21, 2008 No. 309 "About modification and amendments in the resolution of the Ministry of statistics and the analysis of the Republic of Belarus of November 1, 2007 No. 374".
4. This resolution becomes effective since January 5, 2016.
