Based on subitem 8.10 of Item 8 of the Regulations on National Statistical Committee of the Republic of Belarus approved by the Presidential decree of the Republic of Belarus of August 26, 2008 No. 445, National Statistical Committee of the Republic of Belarus DECIDES:
1. Approve on representation of the Ministry of Health:
1.1. form of the state statistical reporting 1 organization (Ministry of Health) "The report of the organization of health care providing medical care in stationary and out-patient conditions" (is attached) and to enact it starting with the report for 2016;
1.2. Instructions for filling of form of the state statistical reporting 1 organization (Ministry of Health) "The report of the organization of health care providing medical care in stationary and out-patient conditions" (are applied) and to enact them starting with the report for 2016.
2. Extend the form of the state statistical reporting specified in subitem 1.1 of Item 1 of this resolution to the state organizations of health care providing medical care in stationary and out-patient conditions, subordinated to republican state bodies, local executive and administrative organs and also the organizations of health care providing medical care in stationary and out-patient conditions shares (share in authorized funds) of which are in state-owned property and are transferred to the control to local executive and administrative organs.
3. Declare invalid the resolution of National Statistical Committee of the Republic of Belarus of October 8, 2014 No. 171 "About approval of form of the state statistical reporting 1 organization (Ministry of Health) "The report of the organization of health care providing medical care in stationary and out-patient conditions" and instructions for its filling" (The national legal Internet portal of the Republic of Belarus, 25.10.2014, 7/2942).
4. This resolution becomes effective since April 4, 2016.
