1. Approve:

1.1. form of the state statistical reporting 4-un "The report on amount of paid services to the population" (is attached) and to enact it starting with the report for January-December, 2016;

1.2. Instructions for filling of form of the state statistical reporting 4-un "The report on amount of paid services to the population" (are applied) and to enact them starting with the report for January-December, 2016.

2. The Item is excluded according to the Resolution of National Statistical Committee of the Republic of Belarus of 16.10.2020 No. 89

3. Extend the form of the state statistical reporting specified in subitem 1.1 of Item 1 of this resolution on legal entities, separate divisions of legal entities according to Item 1 of Instructions on filling of form of the state statistical reporting 4-un "The report on amount of paid services to the population", approved by this resolution.
