Based on subitem 8.10 of Item 8 of the Regulations on National Statistical Committee of the Republic of Belarus approved by the Presidential decree of the Republic of Belarus of August 26, 2008 No. 445, National Statistical Committee of the Republic of Belarus DECIDES:
1. Approve:
1.1. form of the state statistical reporting 4-f (costs) "The cost statement on production and sales of products (works, services)" (is attached) and to enact it starting with the report for January-March, 2017;
1.2. Instructions for filling of form of the state statistical reporting 4-f (costs) "The cost statement on production and sales of products (works, services)" (are applied) and to enact them starting with the report for January-March, 2017.
2. The Item is excluded according to the Resolution of National Statistical Committee of the Republic of Belarus of 24.07.2020 No. 67
3. Extend the form of the state statistical reporting specified in subitem 1.1 of Item 1 of this resolution on legal entities, the separate divisions of legal entities having separate balance, according to Item 1 of Instructions on filling of form of the state statistical reporting 4-f (costs) "The cost statement on production and sales of products (works, services)" approved by this resolution.
4. Declare invalid:
the resolution of National Statistical Committee of the Republic of Belarus of September 29, 2015 No. 123 "About approval of form of the state statistical reporting 4-f (costs) "The cost statement on production and sales of products (works, services)" and instructions for its filling" (The national legal Internet portal of the Republic of Belarus, 08.10. 2015, 7/3238);

5. This resolution becomes effective since April 3, 2017.
