About modification of the Procedures approved by resolutions of the Cabinet of Ministers of Ukraine of July 11, 2007 No. 915 and of December 25, 2015 No. 1130

1. Bring in the Procedure for consideration of the applications about change of name (surname, own name, middle name) physical person approved by the resolution of the Cabinet of Ministers of Ukraine of July 11, 2007 No. 915 (The Official Bulletin of Ukraine, 2007, No. 52, Art. 2115; 2011, No. 84, Art. 3078; 2012, No. 71, Art. 2870; 2015, No. 69, the Art. 2276), and the Procedure for accreditation of subjects of state registration and monitoring of compliance of such subjects to requirements of accreditation approved by the resolution of the Cabinet of Ministers of Ukraine of December 25, 2015 No. 1130 (The Official Bulletin of Ukraine, 2016, No. 2, the Art. 111), changes which are applied.

2. Ceased to be valid according to the Resolution of the Cabinet of Ministers of Ukraine of 28.10.2019 No. 897.

1. According to the procedure of consideration of the applications about the change of name (surname, own name, middle name) physical person approved by the resolution of the Cabinet of Ministers of Ukraine of July 11, 2007 No. 915:
