About approval of form of the certificate of stay on accounting in the Unified state automated register of persons having the right to privileges, and instructions for its filling

According to Item 18 of Rules of accounting of the citizens needing improvement of housing conditions, and provisions of the premises by it in the Ukrainian SSR approved by the resolution of Council of Ministers of USSR and the Ukrainian republican council of labor unions of December 11, 1984 No. 470 (with changes), I order:

1. Approve enclosed:

form of the certificate of stay on accounting in the Unified state automated register of persons having the right to privileges, faces with disability owing to war or the combatant taking into account family members, and also the member of the family of the dead according to the Law of Ukraine "About the status of veterans of war, guarantees of their social protection";

Instruction for its filling.

2. To provide to department of the government public assistance (Muzychenko V.) submission of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine.

3. To impose control of execution of this order on the deputy minister Mushchinin V.

4. This order becomes effective from the date of its official publication.
