According to Item 295.9 of Article 295 and Item 296.9 of Article 296 of Chapter 1 of the Section XIV of the Tax code of Ukraine and Regulations on the Ministry of Finance of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of August 20, 2014 No. 375, I order:

1. Approve Changes in forms of the tax declaration of the payer of the single tax - physical person entrepreneur, the tax declaration of the payer of the single tax of the third group (legal entity), the tax declaration of the payer of the single tax of the fourth group, the Ministries of Finance of Ukraine approved by the order of June 19, 2015 No. 578, registered in the Ministry of Justice of Ukraine on July 7, 2015 at No. 799/27244, which are applied.

2. In accordance with the established procedure to provide to department of tax policy of the Ministry of Finance of Ukraine:

provision of this order on state registration in the Ministry of Justice of Ukraine;

promulgation of this order.

3. This order becomes effective from the date of its official publication.
