About form of the questionnaire of persons which arrived from the countries in which COVID-19 infection cases are registered

(as amended of the Resolution of the Ministry of Health of the Republic of Belarus of 10.04.2020 No. 36)

Based on parts four and the sixth subitem 1.1 of Item 1 of the resolution of Council of Ministers of the Republic of Belarus of March 25, 2020 No. 171 "About measures for prevention of delivery and spread of the infection caused by COVID-19 coronavirus" the Ministry of Health of the Republic of Belarus DECIDES:

1. The Item is excluded according to the Resolution of the Ministry of Health of the Republic of Belarus of 10.04.2020 No. 36

2. Establish form of the questionnaire of person which arrived to the Republic of Belarus from the country in which COVID-19, infection cases according to appendix are registered.

3. This resolution becomes effective after its official publication.
