According to the Procedure for provision and document creation, appointment and payment of compensation to physical persons which provide social services in the leaving on nonprofessional basis approved by the resolution of the Cabinet of Ministers of Ukraine of September 23, 2020 No. 859 "Some questions of appointment and payment of compensation to physical persons which provide social services in leaving on nonprofessional basis" to Item 8 of the Regulations on the Ministry of social policy of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of June 17, 2015 No. 423, I ORDER:

1. Approve enclosed:

the application form about consent to provide social services in leaving on nonprofessional basis;

the application form about consent to receive social services;

form of the act of carrying out inspection of family;

form of the declaration on the income and property condition of family.

2. Make change to the application form about purpose of all types of the public assistance, compensations and privileges approved by the order of the Ministry of social policy of Ukraine of April 21, 2015 No. 441 "About approval of the application form about purpose of all types of the public assistance, compensations and privileges" registered in the Ministry of Justice of Ukraine on April 28, 2015 for No. 475/26920, having excluded Item 9.
