According to Items 5, of 6, 9 Procedures for provision and deprivation of the status of the person deported on national sign, approved by the resolution of the Cabinet of Ministers of Ukraine of November 12, 2014 No. 643, to Item 8 of the Regulations on the Ministry of social policy of Ukraine approved by the Presidential decree of Ukraine of April 6, 2011 No. 389, order:

1. Approve:

1) form of accounting card which is applied;

2) form of the magazine of registration of a statement of provision (deprivation) of the status of person deported on national sign and the identity certificate deported on national sign that is added.

2. To provide to management of the organization of social protection of injured emergency situations (Marushchenko S. V.) submission of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine.

3. This order becomes effective from the date of its official publication.

4. To impose control over the implementation of this order on the deputy minister - the chief of staff of Ivankevich V. V.
