According to Articles 1, 16 Laws of Ukraine "About the state statistics" and Item 8 of the Regulations on the Ministry of social policy of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of June 17, 2015 No. 423 (with changes), for the purpose of reduction of regulatory legal acts in compliance with requirements of the legislation, enhancement of monitoring of provision of social services by subjects which provide social services, order:

1. Approve enclosed:

1) reporting No. 12-sots form (annual) "The report on the organization of provision of social services" (further - form No. 12-sots);

2) Instruction for filling of form of the reporting No. 12-sots (annual) "Report on the organization of provision of social services".

2. And to provision of social services (O. Sulim) to provide to management for elderly people submission of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine.

3. To executive body concerning social protection of the population of the Autonomous Republic of Crimea, to structural divisions concerning social protection of the population of the regional, Kiev and Sevastopol city public administrations to provide collection, control, processing and submission of information on form No. 12-sots to the Ministry of social policy of Ukraine till February 10.
